An exciting opportunity has arisen for two dynam ic, motivated, Nurses or Pharmacy Technicians to join the Swansea Medicines Management Team for Domiciliary Care .
The successful applicant will be part of the integrated, multidisciplinary Team who provide support and education in medicines management, specifically in domiciliary and intermediate care settings. This role aims to improve patient outcomes and minimising harm, including avoiding unscheduled care, admissions, and delays to transfers of care. It is ideal for individuals interested in integrated working and are committed to developing and expanding their skills to achieve the highest quality care and outcomes for patients in their own homes.
The role includes:
providing medicines management interventions to patients within their own home, involving lone working without direct supervision.
liaising with a wide range of Health & Social Care professionals to identify solutions for patients in a multidisciplinary network with a focus on promoting independence, prevention and early intervention .
delivering medicines management training to relevant staff groups.
managing a defined caseload within the speciality, using evidence based/patient centred principles to assess, plan, implement and evaluate interventions, maintaining records.
providing a lead contact between community pharmacy and social services with respect to medicines management issues, including the provision of regular updates, training and visits to community pharmacies to support the Medicines Administration Record scheme.
The post holder must be able to travel around the locality in a timely manner.
