This is an exciting opportunity to join a busy community adult social care team in Nantwich and Rural. Your working base will be Delamere House in Crewe but the team works both in the office and at home.
As a social care assessor you will-
- Carry out basic needs-based assessments with vulnerable people and their carers within agreed procedures, to determine specific individual client needs, identify levels of risk and urgency of response in order to construct an individual Care Plan.
- Implement, co-ordinate and maintain individual Care Plans and act as Care Manager to ensure that services are arranged to meet the identified needs of vulnerable people.
- Monitor, evaluate and review individual Care Plans, identify and respond to significant changes by modifying the Care Plan to ensure that the identified needs continue to be met in the most cost-effective way.
- Negotiate and liaise with other professionals, statutory and voluntary agencies, relatives and carers about care arrangements in order to maintain the best possible service provision for clients and carers.
- Inform clients and carers of the financial implications of care services, undertake financial assessments and encourage clients to access benefits advice in order to maximize income to clients, carers and the department.
- Prepare and maintain client records/systems, both written and electronic to ensure the provision of accurate up to date client related information to the group.

We are looking for an enthusiastic committed individual to join the Nantwich and Rural adult social care team as a part time social care assessor. You will be part of a multi-disciplinary team of social workers, social care assessors, occupational therapists and Lacs who provide information, advice and support to individuals with care and support needs under the Care Act and their careers. Working independently you will hold a caseload and provide assessments of need and support planning for individuals and carers. The team work between home and the office. You will be provided with the appropriate support and training to carry out your role, however some experience of this field is desirable. Cheshire East Council offers opportunities for professional development with opportunities to access apprenticeships to complete qualifications in social care and occupational therapy.
