We have an exciting opportunity for a Clinical Commissioning Nurse Assessor to join our team at Cygnet.
Cygnet was established in 1988. Since then we have developed a wide range of health and social care services for young people and adults with mental health needs, acquired brain injuries, eating disorders, autism and learning disabilities within the UK.
This role is a national role and requires travel to all Cygnet services as required.
Job Purpose & Overview:
- Conduct psychiatric assessments for Cygnet Health Care facilities, covering mental health, learning disabilities, acquired brain injuries, and social care/residential services.
- Evaluate patients referred to Cygnet Health Care, requiring travel to and from NHS and independent hospitals.
- Represent the organization professionally and positively while carrying out assessments.
- Use clinical expertise to assess patients and recommend admission to Cygnet Health Care when appropriate.
- Collaborate with Hospital Managers and Business Relationship Managers to identify suitable patients for admission.
- Develop initial care plans as part of the funding process, outlining clinical and treatment needs.
- Maintain professional relationships within the NHS mental health and learning disability sector.
Key Responsibilities:
Clinical Duties:
- Analyze psychiatric history, assessing past and current mental health risks.
- Conduct patient interviews to evaluate risks, insight, and treatment requirements.
- Perform mental state examinations during assessments.
- Monitor and assess current treatments to determine appropriate interventions.
- Communicate effectively with service users, carers, colleagues, and external agencies.
- Participate in clinical and pipeline meetings to discuss patient assessments.
- Present patient histories and clinical findings to the multidisciplinary team (MDT).
- Work collaboratively as part of the Assessment and Commissioning Team.
- Contribute to discussions and decision-making regarding patient admissions and care plans.
- Maintain accurate and up-to-date written and electronic records in line with Cygnet's and professional standards.
- Engage in training and structured supervision to ensure professional development.
- Adhere to the NMC Code of Ethics, professional standards, and relevant policies.
- Reflect on practice and participate in supervision and performance appraisals.
- Apply clinical knowledge across various service areas for continuous improvement.
- Keep a professional portfolio documenting learning and development.
- Contribute to service planning, clinical pathways, and protocol development.
- Stay informed about national health and social care guidelines and legislation.
- Follow appropriate procedures and policies when working with patients, students, and colleagues.
- Build and maintain respectful professional relationships with clients, their families, and support networks, ensuring culturally sensitive care.
Administrative & Managerial Duties:
- Support the development of assessment services within Cygnet Health Care.
- Ensure compliance with Cygnet's policies on leave, training, and other HR procedures.
- Maintain psychiatric reports, care plans, and documentation.
- Accurately record attendance and relevant statistical data.
- Uphold health and safety policies when visiting external agencies.
- Report and address incidents promptly and appropriately.
- Participate in performance evaluations and service development.
- Stay informed about best practices in assessment and treatment methods.
- Manage time effectively, ensuring punctuality and reliability.
- Maintain necessary resources and manage expenses responsibly.
